Prior to your appointment, you will need to complete the new patient intake paperwork and consent forms. These documents can be completed online via the patient portal. You can also download these forms below and complete them before your visit. 

We will need a copy of a photo ID and the front and back of your insurance card. These images can be uploaded and sent by message in the patient portal, emailed prior to your visit to info@justlivinghealthcare.com, faxed to our practice at 360-230-7299 or these documents can be scanned in person at the time of your visit. 

If you are scheduled for a lactation visit, please have your baby ready for a feeding at the start of the visit.

You will be expected to pay a primary care copay at the time of visit if required by your insurance plan. This copay can be processed by credit card, check or cash. If you are scheduled for a lactation visit, the copay for the baby will be waived as a courtesy to all of our patients.
